Player Requirements

To play soccer at University High School, the student must meet all eligibility requirements of University High School and the WVSSAC.

Prior to tryouts, all players must turn in completed copies of the following forms:

• UHS_Physical_Form.pdf (This form requires a medical professional's signature)
• Medical_Treatment_Form.pdf
• UHS_Insurance_Form.pdf
• Mon_County_Form.pdf

Uniform jerseys and shorts are provided for all field players, but players must provide:

• One pair of white and one pair of black soccer socks.
• A size five soccer ball.
• Proper soccer cleats or soccer turf shoes.
• Shin guards.
• A water bottle.

We welcome all players that have an interest in soccer, have a good work ethic, and are willing to be good teammates and committed members of a team!
